Responsibilities
Responsibilities:
The Pharmacist provides timely, accurate, and complete pharmaceutical care, which includes reviewing medication orders, performing appropriate clinical interventions with prescribers, and providing drug information. Demonstrates competency in patient care for patient population served. The incumbent will also effectively maintain control of daily pharmacy operations and supervise personnel.
Job Duties:
Investigates and completes Medication Event Report. Full Time: Exceeds (3): greater than 2 per month. Meets (2): 1-2 per month. Does not meet (1): less than 1 per month. Part time and per diem will be prorated.
Follows High Risk Medication policies and procedures including appropriate documentation. Exceeds (3): procedures if followed, physician is contacted if patient does not meet criteria and documentation is completed. Does not meet (1): procedure is not followed and/or documentation is not completed.
Effectively maintains control of daily pharmacy operations and supervises personnel. Exceeds (3): consistently aware of the needs of the department and takes specific action to direct staff to meet those needs. Meets (2): directs staff based on daily work flow. Does Not Meet (1): does not consistently provide direction to staff to meet the daily work flow.
Number of orders verified compared against like shifts: Exceeds (3): top quartile. Does Not Meet (1): bottom quartile.
Number of clinical interventions compared against like shifts. Exceeds (3): greater than 15 interventions per month. Meets (2): 15 interventions per month. Does Not Meet (1) less than 15 interventions per month. Part time and per diem will be prorated.
Achieves annual performance goals. Exceeds (3): Achieved greater than or equal to 90% of goals. Does Not Meet (1): less than 80% of goals
Keeps informed of departmental process changes through documentation of reaching minutes, huddles, emails and other communication within one month of information posting or due date assigned. Exceeds (3): greater than or equal to 90% documentation. Does Not Meet (1): less than 80% documentation
Completes Quality Assurance logs as required. Exceeds (3): greater than or equal to 90% documentation. Does Not Meet (1): less than 80% documentation
Qualifications
Education and Experience:
- Bachelor's Degree in Pharmacy
- PharmD preferred
- Post Graduate Residency preferred
- Recent Acute care inpatient hospital experience preferred
License:
- Current California Pharmacy license
- Requires critical thinking skills, decisive judgment, delegation skills and the ability to work with minimal supervision. Must be able to work in complex and dynamic environment and take appropriate actions in emergent situations. Must be able to communicate both written and verbally in an effective, tactful, and diplomatic manner. Must have teamwork and delegation skills and ability to supervise students, interns, residents, technical and clerical staff. Must have the skills to produce accurate work results within established timeframes. Must have ability to provide proper therapeutic drug monitoring and therapeutic interventions. Must have ability to enforce formulary decisions of the medical staff, maintain accurate and complete records, provide instruction and preceptorship on the subject of pharmacotherapeutics, it's use and application. Must have good understanding and ability to apply federal and state regulations and accreditation.
