Job Summary and Responsibilities

As our Licensed Clinical Social Worker, you will coordinate and implement post-discharge plans in close coordination with Case Managers, utilizing the Extended Care Information Network (ECIN).

Every day you will manage patient discharge processes, provide essential advocacy for patients, and facilitate referrals to critical community resources, ensuring seamless transitions of care and comprehensive support.

To be successful in this role, you will be a Licensed Clinical Social Worker with expertise in discharge planning and community resource navigation. Your strong organizational skills, ability to utilize ECIN, and commitment to patient advocacy are essential for effective care coordination.

  • Assesses patient's physical, psychosocial, cultural, and spiritual needs through observation, interview, review of records, and interfacing with interdisciplinary team and caregivers to ensure appropriate referrals.
  • Facilitates discharge planning working with patient, families, and treatment team making any needed referrals/arrangements and documenting actions.
  • Assesses patient/family emotional, social, and financial needs and assists in setting up community resources to meet these needs.
  • Assesses/reassesses patient's clinical and psychosocial status, premorbid status, community services utilized, and diagnosis and treatment plan per Case Management referral.
  • Collaborates with interdisciplinary team to develop and implement holistic, individualized plan of care.
  • Provides support to patients and families who are having difficulty coping effectively with changing medical conditions.

This is an exciting opportunity for a behavioral health clinician who is passionate about meeting patients where they are and providing meaningful, patient-centered mental health care throughout their hospital journey. This position serves as a therapist within the hospital, supporting patients from the moment a behavioral health need is identified through crisis intervention, treatment, and inpatient care. Clinicians will conduct level-of-care assessments, provide crisis intervention and disposition planning, and follow an inpatient therapy caseload, allowing them to work face-to-face with patients throughout different stages of their behavioral health journey.

The role includes facilitating therapeutic groups, providing individual support and interventions, and working closely with providers, nursing, and the interdisciplinary care team to ensure each patient’s mental health needs are heard, understood, and addressed. Job Requirements

Required

  • Master's Degree Soical work
  • Licensed Clinical Social Worker
  • Knowledge of community resources used for discharge planning, hospital operations, excellent communication/presentation skills
  • Knowledge of third party payment systems, Medicare/Medicaid programs
  • Maintains current knowledge base of community services through continuing education
