Responsible for assessing medical inpatient’s psycho-social history, assisting the clinical team in developing a treatment plan, participates in discharge planning process and coordinates follow-up care for patients. Acts as a liaison for family, friends, and therapists and outside agencies. Provides family therapy and education. Provide oversight to coordination of care with social workers and case managers. Responsible for training and mentoring hospital staff. Assists leadership in identifying, developing and implementing process improvement initiatives.


Job Requirements

In addition to bringing humankindness to the workplace each day, qualified candidates will need the following:

Masters Degree in Social  Work, Counseling, Psychology, or Marriage and Family Counseling (Required) 

Two years of psychiatric or mental health experience required.

Previous leadership experience preferred.

Demonstrates clinical and ethical professionalism, conflict resolution and critical thinking skills.

LCSW, LPC, LMFT License

BLS

Sedentary work - prolonged periods of sitting and exert up to 10 lbs. force occasionally

The Behavioral Health department at Holy Cross Hospital – Jordan Valley West is a compassionate and supportive haven for individuals seeking healing and respite. With 26 beds dedicated to our patient’s well-being, we offer a nurturing environment where every individual is treated with utmost care and dignity. Within our unit, we have 4 specialized high acuity beds, ensuring that those in need of intensive care receive the focused attention and comprehensive treatment they require. Additionally, our main unit consists of 22 beds, allowing us to cater to a diverse range of behavioral health needs.

Our friendly and empathetic staff members are the heart of our unit. They create a warm and welcoming environment, fostering a sense of safety and trust for our patients. Our team is comprised of experienced mental health professionals, including psychiatrists, nurses and therapists, who collaboratively ensure comprehensive care. We are dedicated to promoting mental wellness, offering a supportive environment, and helping our patients on their journey to recovery. We believe in the power of compassion and understanding, and we are here to make a positive difference in the lives of those we serve.
