Job Summary and Responsibilities

As a Case Management Professional, you will be a pivotal coordinator and advocate, guiding patients through complex healthcare journeys to optimize outcomes and well-being.

Every day, you will assess patient needs, develop individualized care plans, facilitate seamless transitions, and coordinate resources. You'll educate patients and collaborate with multidisciplinary teams to ensure continuity of care and prevent readmissions.

To be successful in this role, you will possess exceptional assessment and coordination skills, strong communication, and a deep understanding of healthcare systems, with an empathetic approach to patient advocacy.

  • Assures that the rights of the patient/family are respected and maintained.
  • Allows for privacy and modesty in the provision of care.
  • Identifies self by name and title to patient/family.
  • Determines presence/type of Advanced Directive.
  • Reports suspected cases of abuse/neglect, if identified.
  • Assesses the physical, psycho-social, cultural, functional, high risk and needs of the patient and family
Job Requirements

Required

  • Masters in Mental Health
  • Registered intern with BBSE or BBSE eligible
  • Basic Life Support - CPR (BLS) through the American Heart Association 


Preferred

  • One (1) or more years of experience in an Inpatient Psychiatric Hospital

Serving over 3,000 patients annually, the center offers a full complement of services including behavioral evaluations, a chemical recovery program, and inpatient services. Additionally, St. Joseph’s Behavioral Health Center has been recognized as an LGBTQ+ Healthcare Equality High Performer by the Human Rights Campaign Foundation. It is the only psychiatric and substance abuse hospital in the area.
