Job Summary and Responsibilities

As our Transplant Social Worker, you will partner with transplant patients and their support systems to provide comprehensive psychosocial assessments, interventions, and advocacy so they can successfully navigate the transplant journey and achieve continuity of care.

Everyday you will: Monitor the psychosocial stability of patients, assess readiness for transplant, and serve as a vital link between the clinical transplant team and the patient’s home/community environment.  You will be expected to: Perform thorough psychosocial evaluations for potential donors and recipients, document interventions in accordance with regulatory standards, and provide crisis intervention and resource coordination in a compassionate, culturally competent, and tactful manner.  This position is federally mandated by the United Network for Organ Sharing (UNOS) for Medicare-certified solid organ transplant programs, ensuring that our patients receive the holistic support necessary for successful long-term outcomes.

To be successful in this role, you must possess a strong background in clinical social work, excellent crisis management skills, and a comprehensive understanding of the psychosocial complexities inherent in organ transplantation.

Job Requirements

Required Education and Experience

  • Master’s of Social Work from an accredited college or university

Required Licensure and Certifications Preferred Licensure and Certification

  • Licensed Independent Clinical Social Worker (LICSW)

Required Minimum Knowledge, Skills, Abilities, and Training

  • Strong interpersonal and organizational skills
  • Working knowledge of individual and family dynamics and the ability to perform psycho/social assessments.
  • Understanding of systems and how to work within systems.
  • Understanding of the impact and implications of disease and the course of the disease.
