Responsibilities

The Licensed Inpatient Social Worker provides social work interventions for Medical Center patient and family needs. Some responsibilities include but are not limited to: completes a psycho/social assessment for care planning and care transitions, ensuring the appropriate resources are in place to facilitate a safe and timely hospital discharge plan, developing plans for ongoing treatment through services and agencies; providing short-term adjustment counseling to assist with emotional, social, and economic stresses of illness and disability.

Qualifications

Required:  

  • Master's of Social Work from an accredited college or university and Licensed Independent Clinical Social Work (LICSW) license
  • 2 years of Social Work experience in an inpatient setting

Strong interpersonal skills; a working knowledge of individual and family dynamics; strong organizational skills, ability to multitask, an understanding of systems and how to work within systems; an understanding of the impact and implications of disease and the course of disease; excellent communication skills; the ability to perform psycho/social assessments; and a working knowledge of community resources.


Prefer three years in a care coordination, case management or discharge planning role
