Job Summary and Responsibilities
As our Social Worker, you will be a compassionate advocate, providing vital support and guidance to individuals and families facing complex challenges.
Every day, you will conduct psychosocial assessments, develop care plans, provide counseling and crisis intervention, and connect clients to resources. You'll advocate for clients and collaborate with multidisciplinary teams to ensure holistic support and promote well-being.
To be successful in this role, you will possess exceptional interpersonal skills, empathy, strong knowledge of community resources, and proven crisis management abilities, fostering positive change and client empowerment.
- Performs psychosocial evaluations of patients and families, including a detailed psychosocial history.
- Assists the staff in evaluating environmental and psychological implications of illness. Formulates a social casework diagnosis and utilizes this for patient care. Develops plan for social service interventions and goals in collaboration with the patient and/or family and physician.
- Provides direct casework services for patients and families. Implements physician-ordered social service interventions.
- Makes referrals to appropriate community agencies and provides follow-up to assure patients progress.
- Makes referrals to appropriate agencies in cases of suspected child/elder abuse.
- Provides crisis intervention to patients and staff.
Required
- Masters Other Social Work and one year social work experience in a healthcare setting, upon hire and
- Basic Life Support - CPR, within 30 Days and
- Driver's License: CA, upon hire and
- Auto Insurance, upon hire

- Home Health experience
