Job Summary and Responsibilities

As our Case Manager RN, you will coordinate the care and services of patients across the continuum of illness; promote effective utilization; monitor health care resources; and assume a leadership role with the interdisciplinary team to achieve optimal outcomes through the tools of case management.

Every day you will perform a wide variety of high-level patient care activities to include admission assessment, screen for appropriateness, and development of a discharge plan. Timely and appropriate discharges will be accomplished by evaluating the patient's post-discharge needs and the provision of services necessary to meet those needs.

To be successful in this role you must be knowledgeable in community resources and alternate care facilities.

  • Utilizes the nursing process to provide safe therapeutic physician ordered care for patients
  • Assessment- assesses the patient condition at each visit noting significant changes in condition. Conducts Outcome and Assessment Information Sets and other required tools at the appropriate timeframes.
  • Planning- develops a patient specific plan of care in collaboration with the patient and physician. Coordinates timely and accurate preparation of plan of treatment and recertifications as assigned. Makes recommendations that may improve the patient's quality of life
  • Implementation- Oversees or carries out the treatment plan. performs physician ordered tasks maintaining principles of infection control. Treats patients and family members and caregivers with dignity, courtesy and respect
  • Evaluation- Regularly evaluates the needs of the patient. Evaluates patient's progress to goals and prepares the patient for discharge. Completes discharge planning in collaboration with patient, physician, other nurses, social worker and other health care workers as applicable. Participates in patient care conferences
  • Assists with coordination of patient-care activities/scheduling. Collaborates/cooperates to assure effective consistent management of patients. Ensures care coordination occurs with all applicable disciplines. Reports issues following the chain of command
Job Requirements

Required

  • Graduate of an accredited registered nursing program, upon hire or
  • High School GED, upon hire and
  • Registered Nurse: CA, upon hire and
  • Basic Life Support - CPR, upon hire and
  • Driver's License: CA, upon hire and
  • Auto Insurance, upon hire


Preferred

  • Home health experience
