The posted compensation range of $26.76 - $39.81 /hour is a reasonable estimate that extends from the lowest to the highest pay CommonSpirit in good faith believes it might pay for this particular job, based on the circumstances at the time of posting. Job Summary and Responsibilities
As our Community Health Navigator, you will serve as a vital link between underserved communities and essential community resources. You will be instrumental in reducing health disparities by helping clients navigate and access critical health, social, educational, and employment services through a home-visiting model.
Every day you will proactively engage with clients through home visits, meticulously assessing their needs and identifying specific Pathways to care and support. You will connect clients to identified community resources, provide guidance on accessing medical, behavioral health, and social services, and offer practical assistance to overcome logistical obstacles.
To be successful in this role, you will possess a profound understanding of the cultural and geographical nuances of underserved communities. A strong ability to problem-solve, coupled with empathy and resilience, is essential for effectively removing barriers to care and empowering clients.
- Provide Health education to promote health or address barriers to equitable physical and mental health care, including providing instruction or translation for health topics.
- Support the patient through the use of community resources and services to prevent and/or manage disease and to promote the physical and mental health of patients.
- Community Health Navigator to provide information, referrals, obtain authorizations or support to assist patients to: Access health care & understand the health care system. Connect to community resources necessary to promote health and address barriers, including medical interpretation, translation, or transportation services; or to address health-related social needs.
- Outreach and resource coordination to encourage and facilitate use of preventive services
- Help patients enroll or maintain enrollment in assistance programs related to improving health.
- Perform screening and assessment that does not require a license and that assists the patient to connect to appropriate services to improve their health.
Job Requirements
- High School Diploma
- Experience in a healthcare setting working with patients, healthcare services, medicalgroups and payors
- 1-2 years of experience as a community health worker or in a community setting
- Driver's License: CA
Preferred
- Associate Degree in a healthcare related field
