Responsibilities

Pharmacist – St. Bernardine Medical Center

St. Bernardine Medical Center is seeking a skilled and detail-oriented Pharmacist to provide comprehensive pharmaceutical care across inpatient services. In this vital role, you will ensure safe medication practices, participate in performance improvement initiatives, and serve as a trusted resource for clinical staff and providers.

Key Responsibilities:

  • Review, process, and dispense medications and IV therapies with precision and in compliance with all hospital policies and regulatory guidelines
  • Provide direct clinical consultation and medication therapy recommendations to physicians and nursing teams
  • Supervise and support pharmacy technicians in compounding, repackaging, and medication distribution
  • Monitor patient medication profiles for drug interactions, allergies, and contraindications
  • Perform pharmacokinetic calculations and adjust dosages as appropriate for patient age, weight, and condition
  • Support formulary compliance, therapeutic interchange, and clinical pharmacy initiatives
  • Participate in department performance improvement projects, medication use evaluations, and adverse drug reaction monitoring
  • Ensure accurate and timely documentation within the EMR and pharmacy systems
  • Maintain compliance with controlled substance protocols, regulatory standards, and documentation requirements

Qualifications
  • 1-2 years experience in Hospital Pharmacy, or completion of a Pharmacy Residency program preferred 
  • Experience in unit-dosing, IV admixtures, pharmacokinetics and TPN desire
  • Graduate of a School of Pharmacy, required Pharm D. desirable
  • Basic knowledge of computers desirable
  • California Pharmacist License
