Responsibilities

Position Summary:

The incumbent as a member of the multi-disciplinary team provides the following services to patients and/or families: psychosocial assessmentcrisis intervention counseling exploration of effective coping skills crisis intervention education and coordination of the patients progress through the continuum of care. In addition the incumbent provides education consultation and support to hospital staff.


Qualifications

Minimum Qualifications:

  • This position requires at least 1 year acute hospital experience or a combination of hospital and other clinical experience
  • Masters degree in Social Work
  • Successful attainment of designee status by Sacramento County for 5150 application for patients.
  • Excellent customer service and presentation skills are a must
  • Strong interpersonal and written communication skills are essential
  • Demonstrated ability to apply analytical and problem solving skills
  • Demonstrated ability to manage multiple tasks or projects effectively
  • Ability to work independently as needed with a high degree of detail orientation. Ability to work efficiently in a fast-paced environment with changing prioritie
