Responsibilities
The Case Management Coordinator – Non RN performs activities delegated by RN Case Manager to implement the integrated discharge plan developed by the Case Manager. The incumbent uses effective interpersonal skills in all interactions involved with the position including the explanation of services and benefits as directed by the RN Case Manager.
This position assists with the process of efficiently and effectively coordinating movement through the continuum of care to ensure timely discharge. The Case Management Coordinator assists in implementing the physician's plan for discharge through effective communication with the case management team, care providers, patients, families, and external vendors.
REQUIREMENTS:
Direct patient care or health education in an inpatient, skilled nursing, home health, rehab, or related setting.  Hospital experience, preferred.
Language skills: Excellent oral and written communication skills. Excellent interpersonal skills and time management skills, ability to work effectively in a fast pace environment, with rapidly shifting priorities and competing demands. Ability to work independently with a minimum of direction. Ability to exercise discretion and make independent judgments, seeking review when decisions represent significant departure from established guidelines.
PC skills, word processing, knowledge of MIDAS, Cerner, and MS4A.A. Degree in Business/Health-Related field or equivalent experience in a health care setting


Qualifications
