Join our compassionate team at Mercy Medical Center and provide vital support to patients and families navigating challenging times.
As a Social Worker, you'll play a crucial role in our Home Health/Hospice program. You'll conduct psychosocial assessments, develop individualized treatment plans, and connect patients with essential community resources. Your expertise will empower patients and their loved ones to cope with illness, loss, and the complexities of healthcare.
What you'll do:
- Conduct comprehensive psychosocial assessments to understand patient and family needs.
- Develop and implement tailored treatment plans aligned with Medicare and other payer guidelines.
- Provide short-term therapy, crisis intervention, and grief counseling.
- Serve as a vital link to community resources, ensuring patients receive the support they need.
- Collaborate with a multidisciplinary team to provide holistic, patient-centered care.
LCSW $38.72 - $58.55; MSW $35.20 - $53.23
Qualifications
What you'll need:
- LCSW or LMSW licensure from the State of Oregon
- Healthcare experience preferred
- Master’s degree from school of social work accredited by the Council of Social Work Education.
