Roseburg is located on Interstate 5 in Southern Oregon's scenic Douglas County. The community is nestled in a lush forested region known as the 100 Valleys of the Umpqua. Responsibilities
As a Community Health Worker for the Foundation you will address health-related social needs of patients as well as to provide connection to resources for hospital staff.
Essential Duties:
- Address immediate patient needs before hospital discharge
- Connect patients with resources to alleviate economic disparities
- Assist patiens in applying for public benefits
- Facilitate access to support services and community resources and tools for self-management support
- Provide education and training to promote self-management, transportation and housing
- Disseminate information to employees about community resources
Shift: 8am - 5pm
Schedule: Monday - Friday
Qualifications
License/Certifications:
- CHW required within 6 months of hire
Experience:
- 1 year healthcare and/or community-based work experience that would demonstrate attainment of the requisite job knowledge/abilities OR volunteer service in these areas of marginalized people and/or healthcare related services focused on the social determinants of health
