Job Summary and Responsibilities

This vital role supports our RN Care Coordinators and Social Workers by managing critical, time-sensitive functions that ensure seamless patient transitions and optimal outcomes. What You'll Do:
- Facilitate Post-Acute Care: Manage timely referrals, communicate with agencies, and ensure follow-up appointments to prevent readmissions and mitigate denials.
- Patient & Family Liaison: Directly communicate with patients and families, providing essential information and securing necessary documentation.
- Interdisciplinary Collaboration: Work closely with healthcare teams, patients, and external stakeholders to progress care efficiently.
- Ensure Compliance: Uphold strict confidentiality (HIPAA), deliver regulatory notifications, and identify process improvements.
- Administrative Support: Manage transportation referrals, insurance requests, authorizations, and crucial communications for licensed staff.
Schedule: Monday - Friday 8am - 4:30pm
Job Requirements
What You'll Bring:
- Required Education/Experience:
- Associate's Degree (Business/Health) OR High School Diploma/GED + current healthcare professional license/certification (MA/CNA/EMT).
- Medical terminology expertise (certificate or demonstrated knowledge).
- Intermediate to advanced computer skills (Microsoft Office/Google Workspace).
- Required Skills:
- Excellent customer service and communication (written/interpersonal).
- Strong analytical, problem-solving, and detail orientation.
- Ability to manage multiple tasks independently in a fast-paced setting.
Preferred Qualifications Welcome: 3+ years healthcare experience, Bachelor's degree, experience in demanding patient care environments, relevant certifications.
