Job Summary and Responsibilities
As our Physical Therapist, you will help patients rebuild or maintain their physical wellbeing so they can function at their best level, reduce pain, and improve their quality of life.
Every day you will perform initial evaluations, develop and update individualized plans of care, and provide a variety of direct treatments. You will be expected to consistently meet patient care productivity standards, collaborate with the interdisciplinary team and families, and promote a positive customer service environment.
To be successful in this role, you must possess strong age-appropriate competency skills, excellent communication abilities, and a collaborative, patient-centered approach to care.
- Conducts initial evaluations on assigned inpatients within 24 hours of receipt of order, ensuring they are completed within 48 hours
- Performs initial evaluations for newly referred patients to determine the primary and secondary problems involved.
- Collects baseline data and determines the appropriateness of the prescribed care
- Develops a patient treatment plan based on the information obtained from the initial evaluation, including modalities/procedures to be used, treatment objectives, frequency of patient visits, and time frame for short-term goals
- Re-assesses the patient's progress and revises the treatment plan on a bi-weekly basis or sooner
- Completes documentation in the patient's chart which includes the subjective and objective responses and any anticipated changes in the treatment plan. Reviews patient care plan daily and adjusts it appropriately
Job Requirements
Required:
- Graduate of a school of physical therapy approved by the American Physical Therapy Association
- Physical Therapist: CA
- Basic Life Support - through an American Heart Association provider
