Job Summary and Responsibilities

Job Summary:

The Pharmacist Resident is an active participant in an intensive 12 month post-graduate year one (PGY1) training program to develop extensive knowledge and clinical skills used to provide effective pharmaceutical care.  The pharmacist resident functions as a clinical pharmacist, participating in clinical, operational, and administrative activities designed to assist in applying formal theoretical education and research to clinical patient situations. The resident is responsible for medication-related care of patients with a wide range of conditions, is eligible for board certification, and eligible for postgraduate year two (PGY2) pharmacy residency training. 

Essential Key Job Responsibilities:

  • Participate in all aspects of comprehensive clinical pharmacy services including rounds with medical teams, pharmacokinetic consultations, parenteral nutrition consultations, anticoagulation management, antimicrobial stewardship monitoring, medication reconciliation, discharge counseling, documentation of clinical interventions in the identified data base, and compliance with clinical pathways and protocols
  • Performs all activities associated with meeting the goals and objectives of the American Society of Health-System Pharmacists (ASHP) accredited PGY1 residency program.
  • Process prescriber orders in the identified pharmacy information systems in an effective, efficient, timely, safe and accurate manner to ensure the appropriateness of the indication, dose/frequency, route, interactions, dose adjustment, physical and therapeutic incompatibilities, side effects, duplicate therapies, and cost-effectiveness. Evaluate need for all non-formulary medication required.
  • Provide direct oversight and timely and accurate verification of final product for all aseptic parenteral and non-parenteral medication preparation, dispensing and distribution in compliance with state, federal statutes and hospital procedures.
  • Ensure doses are filled and delivered within established departmental guidelines in a timely and safe manner.
  • Identify and evaluate adverse drug reactions and medication errors
  • Enhance communication and drug information provision to healthcare workers and patients
  • Participate in committee activities and initiatives related to practice area
  • Respond to code situations
  • Provide teaching, supervision, and serve as a role model to technicians and students
  • Other duties as assigned by management


Job Requirements

Minimum Qualifications:

  • Graduation from an Accreditation Council for Pharmacy Education (ACPE) approved school of pharmacy with a BS, MS, or Pharm D degree, including didactic courses and experiential rotations in the clinical aspects of pharmaceutical care
  • Active Pharmacy Intern license if required by the state
  • BLS-CPR (Must be through the American Heart Association)
  • Obtain Active Pharmacist License in good standing in the state in which the employee works required within 3 months
  • Excellent written and verbal communication skills
  • Knowledge of regulatory requirements applicable to the pharmacy profession
  • Knowledge of the principles, practices, standards and techniques relating to the compounding and dispensing of medications
  • Knowledge of the principles and practices of pharmaceutical practice within the acute and ambulatory settings
  • Skills in multidisciplinary team involvement with direct patient care
  • Computer applications such as Microsoft Office

Preferred Qualifications:

  • ACLS (Must be through the American Heart Association)
