Job Summary and Responsibilities
The Nurse Navigator is assigned to a population of patients and provides individualized and comprehensive care to reduce barriers coordinate diagnostic and treatment efforts and ensure prompt access to care. Applying the nursing process the Nurse Navigator performs clinical assessments; education on disease process plan of care and treatment modalities; and symptom management to support patients through the complexity of their care continuum. The Nurse Navigator collaborates and communicates effectively with the multidisciplinary team to effectively coordinate patient care ensure continuity of care and create a comfortable and exceptional patient experience. The Nurse Navigator initiates referrals to appropriate inter-disciplinary teams for physical psycho-social and spiritual support through all stages of the continuum of care.

As our Oncology Care Coordinator, you'll work with physicians, other registered nurses, specialized departments, and community resources. This provides patients with the education and resources needed to build a care plan at discharge.
Every day, you will build trust in these interactions by showing compassion, using your critical thinking skills, and remembering that each patient requires a custom care plan tailored to their needs. You will support patients by identifying and removing patient care barriers that will regress their healing.
To be successful in this role, your talent for creative thinking will be vital, shining through as you develop enhanced discharge plans that yield optimal outcomes for each patient in case management.
- Essential Functions and Responsibilities
- Provides coordination of care and support for patients, families and caregivers throughout the continuum of care.
- Provides education for patient, caregiver and family on disease process, plan of care, treatment, symptom management and survivorship.
- Effectively communicates with patient, caregiver, families and multidisciplinary healthcare teams.
- Provides nursing assessment through a process of gathering pertinent patient's medical history and review of presenting symptoms.
- Demonstrates professionalism in the workplace with patients, families, healthcare team and community.
Job Requirements
Required
- Bachelors Of Science Nursing or or equivalent combination of education and experience
- 4-6 years Minimum of 4 years clinical nursing experience, of which at least 2 years must be oncology nursing
- Registered Nurse: CA
- Basic Life Support - CPR
- Cancer Chemotherapy Provider, within 90 Days and
- Oncology Certified Nurse, within 24 - months
