Job Summary and Responsibilities

As our Licensed Vocational Nurse (LVN), you will provide vital direct patient care, significantly contributing to patient well-being and recovery as an integral part of our compassionate healthcare team.

Every day, you will assess patient conditions, administer medications, perform prescribed treatments, and meticulously document care. You will collaborate with RNs and physicians to implement individualized care plans, ensuring a seamless and high-quality patient experience.

To be successful in this role, you will demonstrate exceptional clinical skills, a strong commitment to patient safety, and excellent communication. Your compassionate demeanor, attention to detail, and ability to prioritize in a fast-paced environment are essential.

  • Utilizes the nursing process for planning and delivery of direct and indirect patient care. Evaluates effectiveness of nursing interventions and updates Plan of Care as appropriate to patient condition/need
  • Performs documentation in patient records that is timely, accurate, and concise
  • Provides appropriate support at time of death
  • Performs efficiently in emergency situations, following established protocols, appropriate communication and documentation
  • Communicates with Interdisciplinary Team members regarding patient status and changes to Plan of Care per Hospice policy
  • Assumes responsibility for professional growth by taking advantage of the available opportunities. Participates in peer review and staff development, which includes unit and quality process improvement activities

Job Requirements

Required

  • Graduate of a BRN approved school of vocational nursing required
  • 1-3 years Minimum 2 years acute care experience within previous 5 years required
  • Licensed Vocational Nurse: CA
  • Basic Life Support - BLS through an American Heart Association provider
  • Driver's License: CA

Preferred

  • Auto Insurance
  • Hospice LVN experience
