Dignity Health Connected Living has been serving seniors in Shasta County since 1968. Our programs have been designed for seniors and people with disabilities. We are dedicated to meeting each participants physical mental and spiritual needs and we offer a variety of high-quality affordable health care services in a compassionate environment.


Responsibilities

Position Summary:

The Health Coach is an important clinical participant in the Integrated Teamlet Model for practice transformation. They help members navigate, communicate and advocate for their health needs, and assist with health self-management activities. Health Coaches help members develop a self-management plan that is individualized, strength and recovery based.


Qualifications

Minimum Requirements:

  • 2-4 years experience with care coordination, case management or discharge planning within a physician practice, medical facility, hospital or health organization.
  • Bachelor degree in health care related field with case management level experience.
