The posted compensation range of $24.00 - $28.15 /hour is a reasonable estimate that extends from the lowest to the highest pay CommonSpirit in good faith believes it might pay for this particular job, based on the circumstances at the time of posting. Job Summary and Responsibilities
As our Community Health Worker, you will serve as a vital link between underserved communities and essential community resources. You will be instrumental in reducing health disparities by helping clients navigate and access critical health, social, educational, and employment services through a home-visiting model.
Every day you will proactively engage with clients through home visits, meticulously assessing their needs and identifying specific Pathways to care and support. You will connect clients to identified community resources, provide guidance on accessing medical, behavioral health, and social services, and offer practical assistance to overcome logistical obstacles.
To be successful in this role, you will possess a profound understanding of the cultural and geographical nuances of underserved communities. A strong ability to problem-solve, coupled with empathy and resilience, is essential for effectively removing barriers to care and empowering clients.
- Advocates for individual and community health needs 1.2 Builds individual and community capacity by increasing health knowledge and self-sufficiency through outreach, community education, informal counseling, social support and advocacy
- Conducts standardized client intake and risk assessments
- Based on assessments, develops and continually updates plan of care based on client-identified health and social services goals
- Participates in the ongoing assessment of client needs via in-person, home, in-office, virtual, and/or telehealth appointments, as appropriate. Similarly, follow-up with clients via telephone, home, or other locations where the client can be found, when appropriate.
- Collaborates with health providers to develop an integrated, holistic solution to help client access services and improve client health outcomes which includes working closely and cooperatively with other personnel assigned to the same client, including medical providers, behavioral health providers, housing coordinators, legal advocates
- Identifies and helps to enroll eligible individuals in federal, state and local private and non-profit health and human services programs which includes assisting clients in completing a variety of applications and forms
Job Requirements
Required
- High School Graduate
- One (1) or more years of experience in social services or care coordination, specifically serving older adults, behavior health clients or adults with complex chronic conditions
- Driver's License: CA
Preferred
- Community health worker training or certification
