Job Summary and Responsibilities
As our Cardiovascular Nurse Coordinator, you will utilize discretion and independent judgment to facilitate seamless care rendered by the heart program.
Every day you will coordinate patients through the entire care continuum, from referral through follow-up care, specifically for those who are candidates for cardiovascular related therapies.
To be successful in this role you will demonstrate exceptional organizational and communication skills, employing independent judgment to ensure optimal patient outcomes and a streamlined experience. Your critical role in facilitating comprehensive and continuous cardiovascular care will be paramount to the success of the heart program.
- Cardiovascular Nurse Coordinator facilitates a team approach and collaborates with the respective team members to establish a plan of care starting from initial interventional evaluation and following the continuum of care including pre-procedure, post-procedure, and continued follow-up post care working with the patient and family to provide teaching, education and support throughout the process.
- Serves as a liaison among physicians, staff, patients, families, and payers to ensure effective utilization of resources and coordination of care. Ensures maintenance of established electronic database, and submits patient demographic, medical and progress report information within designated time frames as required by local, state, and federal agencies. Participates in the department quality improvement program and strives to optimize patient outcomes through patient education and clinical expertise.
- Assesses the medical record to identify possible clinical documentation opportunities which establish medical necessity and appropriate level of care.
- Facilitates accurate clinical documentation that supports severity of illness, risk of mortality, level of services provided, quality outcomes and timely completion of the health record documentation.
- Reviews Heart Center patients (PCI /STEMI / GTWG-CAD) for appropriate care and coordination of services. Abstract, enters data timely and appropriately for measuring documentation outcomes. Gathers, reviews and submits to registries as required such as SRC STEMI/GTWG-CAD Registry, AICD Registry, Cath/PCI Registry and etc.
- Interacts in written form and verbally, with physicians, nursing, ancillary care providers, and coders to obtain compliant documentation consistent with care provided.
Required
2 years of experience in the acute care setting, as a registered nurse, required.

- Bachelors Of Science Nursing
- Prior service line/ or healthcare experience in Neurovascular, Stroke, Trauma, Cardiovascular, or Orthopedic programs experience, upon hire
