Job Summary and Responsibilities
As our PRN Care Coordination Assistant you will play a pivotal role in ensuring seamless patient transitions and optimal care delivery. This includes managing crucial post-acute care referrals, facilitating direct communication with patients and families, and collaborating with a diverse network of healthcare stakeholders and post-acute agencies. Your contributions will directly impact patient length of stay, mitigate denials, and uphold our commitment to regulatory compliance and exceptional service within the healthcare sector.
Every day you will collaborate closely with RNCCs and Social Workers, meticulously manage timely post-acute care referrals to optimize patient flow and prevent readmissions, and ensure precise communication within interdisciplinary teams. You will diligently deliver required patient notifications, secure essential documentation, and expertly manage transportation referrals. Furthermore, you will prioritize, manage, and document critical communications related to insurance requests, authorizations, and physician recommendations, while adhering strictly to confidentiality policies and HIPAA regulations. Your proactive approach will also contribute to identifying opportunities for continuous process improvement.
To be successful in this role, you will possess a minimum of three years of preferred experience in healthcare, specifically in care coordination, utilization review, or discharge planning. A strong working knowledge of medical terminology is essential, coupled with intermediate to advanced computer skills (Microsoft Office, Google Workspace). You will demonstrate exceptional customer service, presentation, analytical, and problem-solving abilities, alongside strong interpersonal and written communication skills. The ideal candidate will thrive in a fast-paced environment, exhibit excellent attention to detail, and possess the ability to effectively manage multiple tasks and projects independently, ensuring compliance with all hospital policies and federal/state regulations.
Job RequirementsRequired
- Associate (two-year) Degree in Business/Health-Related field, or
- High School Graduate General Studies, or
- High School GED General Studies, and
- Working knowledge of medical terminology.
- Excellent customer service and presentation skills.
- Strong interpersonal and written communication skills.
- Demonstrated ability to apply analytical and problem solving skills.
- Demonstrated ability to manage multiple tasks or projects effectively.
- Ability to work independently as needed with a high degree of detail orientation.
- Ability to work efficiently in a fast-paced environment with changing priorities.
- Medical terminology certificate or ability to demonstrate knowledge of medical terminology.
Preferred
- At least 3 years previous experience in health care, care coordination, utilization review, discharge planning or similar healthcare setting. and
- Bachelors Other or BSW
- Experience supporting clinicians in demanding and fast paced patient care environments
- Licensed Practical Nurse: AZ, or
- Certified Clinical Medical Assistant, or
- Certified Medical Assistant AAMA, or
- Certified Medical Assistant NHA, or
- National Certified Medical Assistant NCCT, or
- National Registered/Certified Medical Assistant NAHP, or
- Registered Medical Assistant AMT,
- Registered Medical Assistant, or
- Cert Med Asst - MMCI, or
- Certified Nursing Assistant: AZ, or
- Emergency Medical Technician: CA
- Intermediate to advanced computer skills required to include Microsoft Office and/or Google Workspace applications including Word, Docs, Excel, Sheets, typing, and data entry skills.
