Job Summary and Responsibilities
The Specialty Nurse Navigator plays a crucial role in patient care, offering comprehensive in-person and telehealth management and oversight for a highly complex patient population. The Specialty Nurse navigator is assigned to a complex population of patients providing individualized and comprehensive care across multifunctional groups to reduce barriers, coordinate diagnostic, pharmacological and treatment efforts while promoting and ensuring prompt access to care including home health, biologics, hospital, ER and clinic support. The specialty nurse navigator will anticipate and direct the care of the patient through evaluation, risk reduction and advanced clinical decision making. Applying the nursing process, the specialty Nurse Navigator performs clinical assessments; education on disease process, plan of care and treatment modalities to include a complex pharmacological regimen and symptom management to support patients through the complexity of their care continuum to end of life. The specialty Nurse Navigator collaborates and communicates effectively with the multidisciplinary team to effectively coordinate patient care, ensure continuity of care and create a comfortable and exceptional patient experience. There is a heavy navigation of biologics, home health and pharmacological treatment for this patient population, requiring the Specialty nurse navigator to have a solid and comprehensive understanding of insurance and biologic agencies whom you partner with for the delivery and continuity of care for this complex population. The specialty Nurse Navigator initiates referrals to appropriate interdisciplinary teams for physical, psycho-social and spiritual support through all stages of the continuum of care.
- Coordinate and Manage Patient Care Plans – Oversee the continuum of care for advanced lung disease patients, including clinic, pharmacy, home health, and telehealth pathways, ensuring quality, safety, and timeliness.
- Conduct Comprehensive Patient Assessments – Perform in-person and virtual evaluations of medical, physical, psychological, and psychosocial status, triaging routine, urgent, and emergent needs appropriately.
- Facilitate Communication and Collaboration – Serve as a liaison among patients, caregivers, multidisciplinary healthcare teams, biologics providers, and community resources to promote optimal outcomes.
- Provide Patient and Caregiver Education – Deliver individualized education on disease processes, treatment plans, medication management, symptom control, survivorship, and self-advocacy skills.
- Navigate Insurance and Authorization Processes – Manage prior authorizations, insurance approvals, and patient assistance programs to ensure uninterrupted access to life-saving pharmacologic agents.
Job Requirements
MINIMUM
Bachelors of Nursing
3 years of experience as an RN in an inpatient or outpatient setting
or
1 year RN with 3 years experience coordinating patient care, navigating the healthcare system, providing psychosocial support to ICU, tele and/or complex patients in a healthcare setting, home health, clinic and/or pharmacy
AZ RN license
American Heart Association Basic Life Support - CPR (BLS-CPR)
Strong knowledge of medical terminology
Computer skills, EMR documentation, Word, Excel, Record/Verify system, work experience: patient education/interventions, symptom management.
