Responsibilities
Provides psychosocial assessments discharge planning/coordination community referrals crisis intervention and supportive counseling. This position will have integral role in leading Multi-disciplinary Team based care model in the Dignity Health Medical Group. The Social Worker will plan organize and implement social work services for patients including determining referrals for abuse and neglect of adults and children assessing and coordinating services for psychiatric/ suicidal patients and determining safe discharges for vulnerable patient populations. This position will assist in instructing and educating doctoral residents regarding community resources mandated reporting and providing additional education as needed.
- Provides psychosocial assessments and support as appropriate to diverse populations.
- Providers supportive counseling to patient and families, crisis intervention, and conflict resolution
- Actively participates or leads multidisciplinary team meetings.
- Provides community referrals to patients and families and the multidisciplinary team
- Provides crisis intervention and supportive counseling for patients and families such as awareness of EMTALA, Abuse/neglect, Terminal Illnesses, Adjustment to Disability.
- Provides crisis intervention and supportive counseling for patients and families such as Chronic Illnesses, Cultural/Spiritual, Pain Management and Fetal Demise.
- Provides both educational and primary community services coordination in assigned areas for abuse, neglect, and domestic violence situations involving babies, children, and adults.
- Completes screening, assessments, and reassessments according to department guidelines
- Demonstrates all elements of a comprehensive and/or brief assessment according to Case Management Department Assessment Policy # 1040
Qualifications
MINIMUM
Masters Degree in Social Work from an accredited university required.
One year medical or hospital social work experience, including post-masters and/or supervised internship required.
LMSW license from the AZ Board of Behavioral Health Examiners
Experience in psychosocial assessments and safety planning, with a wide variety of populations.
Efficient documentation and communication skills.
Knowledge of Social Work Code of Ethics, National Association of Social Work (NASW) Standards of Practice.
PREFERRED
Two years post-masters experience: medical, hospital social work, or outpatient primary care setting experience
Knowledge of best practice in clinical social work and regulation standards within a healthcare setting
Experience with Allscripts and Cerner
