The posted compensation range of $23.00 - $23.00 /hour is a reasonable estimate that extends from the lowest to the highest pay CommonSpirit in good faith believes it might pay for this particular job, based on the circumstances at the time of posting. CommonSpirit may ultimately pay more or less than the posted range as permitted by law.
Job Summary and Responsibilities
As our Community Health Worker, you will be a cornerstone in driving healthy behaviors and improving health equity within our community, especially for high-risk and underserved populations. You will lead outreach efforts, implement vital health programs, and act as a crucial link between individuals and essential healthcare services in our Foundation Funded Employees department.
Every day you will conduct outreach for various health initiatives, advising individuals and groups on critical issues like diabetes, obesity, prevention, immunization, and childcare. You will identify and contact members of high-risk groups (e.g., minorities, low-income, pregnant women), providing one-on-one coaching and cultural mediation related to health services.
To be successful in this role, you will have a High School GED or High School Graduate qualification. You must be computer literate and possess strong customer service orientation with the ability to handle highly confidential information. Required skills include the ability to work independently, set priorities, manage work within timeframes, and communicate effectively, both verbally and in writing, with a positive mental attitude.
- Advise clients or community groups on issues related to diabetes and obesity, improving general health, diet and exercise, prevention, sanitation or hygiene, self care, social issues, education, childcare or problem solving.
- Act as a resource to all clients, providers and the community. Attend community meetings and chealth fairs to understand community issues and build relationships with community members.
- Distribute flyers, brochures, or other informational or educational documents to inform members of a targeted community. Refer community members to needed health services.
- Advise clients or community groups to ensure parental understanding of the importance of childhood immunizations and how to access immunization services
- Advocate for individual or community health needs with government agencies or health service providers.
Required
- High School Diploma/GED
- Experience working in the community
- Experience with Community Organizations and Home Health follow-up
- Ability to work independently, set priorities and manage work within timeframes, read and analyze technical material, communicate effectively verbally and in writing, positive mental attitude
- Strong customer service orientation and the ability to handle highly confidential information essential
- Must be computer literate
- Will complete Community Health Worker (CHW) or Promotoras training
- Occasional Weekends

- Teaching experience
- Bilingual (English/Spanish)
