Responsibilities
  • Social Work Assessments for physical acute rehab
  • Facilitate patient/family conferences with the physical rehab team as needed 
  • Educate patient/family of the physical rehabilitation program
  • Advocate for patients to obtain ARU authorization extensions, address insurance denials, and facilitate insurance appeals
  • Assist with discharge planning (ordering DME, HH/OP referrals), etc.
  • Assist in post acute placement on complex discharges and engagement of appropriate agencies of community resources

Qualifications
  • Requires California MSW 
  • LCSW preferred
  • Two - three (2-3) years clinical experience in acute physical rehabilitation and/or medical/surgical
