Job Summary and Responsibilities
Do you love helping people navigate life’s toughest moments? Join our team as a Social Worker and make a real impact for patients and families in the hospital, clinics, or mental health centers. You’ll provide support, guidance, and resources, helping patients get the care they need while connecting them to the community services that make life easier.
What You’ll Do:
- Offer crisis support, counseling, and resource referrals for patients and families 
- Help plan safe discharges and transitions of care 
- Coordinate with healthcare teams to keep patients supported and informed 
- Connect patients to community resources for housing, food, transportation, and more 
Shifts Available:
- M-F Full Time Days
- PRN/Casual Days and Weekends
- Part Time Days, Every 5th Weekend
- Sat/Sun/Mon or Fri/Sat/Sun
***depending on shift, eligible for up to a $5,000 sign-on bonus, $10,000 in student loan reimbursement, and shift differential***
Job Requirements
Bachelor's BSW or Master's Degree MSW (or willing to enter a Master's Degree program) from a school of Social Work accredited by the Council of Social Work Education
