Job Summary and Responsibilities
As a Care Coordination Assistant, you'll be instrumental in ensuring our patients receive seamless post-acute care. You'll be the go-to person for:
- Connecting Patients to Care: Managing and expediting post-acute care referrals (think rehab facilities, home health agencies) to ensure timely transitions.
- Patient & Family Support: Directly communicating with patients and their families, offering guidance, and gathering necessary signatures.
- Team Collaboration: Working closely with nurses, doctors, and other team members to keep patient care moving forward.
- Scheduling & Follow-Up: Arranging crucial follow-up appointments to prevent readmissions and support long-term health.
- Essential Documentation: Delivering important patient notifications and ensuring all paperwork meets regulatory standards.
- Smooth Transfers: Coordinating transportation referrals to get patients where they need to go.
- Confidentiality Champion: Handling sensitive patient information with the utmost care, adhering to HIPAA and other privacy regulations.
Job Requirements
Experience: At least 3 years of experience in healthcare, care coordination, utilization review, discharge planning, or a similar fast-paced clinical setting.
Education:
Associate's Degree in a Business/Health-Related field OR
High School Diploma with a Medical Assistant or Certified Nursing Assistant certificate/experience in a healthcare setting.
Bonus Points For: Bachelor's degree, LVN/LPN, or BSW.
