We're looking for a dynamic and dedicated Community Health Navigator II to join our team and make a tangible difference in people's lives!

Responsibilities: 

  •  Proactively engaging with community members and their families to share essential information and offer support, acting as a friendly and knowledgeable guide.
  • Advocating for community members, ensuring their needs are heard, and that they receive timely and appropriate access to care and services.
  • Guiding individuals to a wide range of relevant resources, from financial assistance programs and transportation services to essential health, mental health, and social services.
  • Regularly participating in important public health meetings, contributing your insights and expertise to broader community health initiatives.

Job Requirements
  • Bachelor's Degree in Public Health, or other related field required 
  • Minimum of one (1) year of data entry experience required. 
  • Six (6) months of experience working with the public or with community groups. 
  • CA Driver's License required.
  • Fire Safety (FIRESAFETY) within 90 days is required
