The posted compensation range of $43.04 - $62.41 /hour is a reasonable estimate that extends from the lowest to the highest pay CommonSpirit in good faith believes it might pay for this particular job, based on the circumstances at the time of posting. Responsibilities
At CommonSpirit Health at Home, we strive to embody our mission of delivering hope and healing to those we serve. As the Director of Health at Home Innovation you will contribute to the planning, development and implementation of solutions and innovative care at home delivery models that demonstrate meaningful and measurable value to the patient, family, health system, physician, payers and communities we serve. Other responsibilities include:
- Leads, supports and monitors ongoing care at home innovation across the care continuum. This includes participation in program development, facility network development and relations, physician network development and relations; patient tracking across the continuum of care; and decisions regarding homecare and hospice alternatives to acute care which consider quality, cost, patient experience and outcomes.
- Works with key internal and external stakeholders, including all organization service lines including but not limited to, finance, revenue cycle, legal, care management, physician enterprise, strategy, corporate responsibility and population health team members, and other key clinical and business leaders per market.
- Conducts ongoing healthcare analyses relating to demographics, demand, competition, and growth opportunities in support of care at home innovation models. Provides analysis of external and internal data sources and develops related presentations to support senior-level decision-making. Runs complex analyses, forecasts and data modes.
- Coordinates data sharing between the clinical, operations and business development teams, legal, corporate responsibility, information technology, finance, revenue cycle, and other population health management stakeholders. Monitors performance in these programs and provides updates to leadership. Develop creative reimbursement options with hospital partners and payers as the organization moves more toward value based reimbursement, while working with compliance and legal.
- Develops close working relationships and trust within the Organization’s culture and structure, to meet or exceed the home health strategy, care management at all levels, and population health goals set forth by the organization, understanding the complexity as healthcare and elevates the capabilities, value, awareness and importance of the Organization’s roles and opportunities.
- Serves as adviser to operations across the continuum of care for all new CMS innovation payment programs.
- All other duties as assigned.

Qualifications
Candidates must reside in one of the following approved remote worker states: AR, AZ, CO, FL, GA, IA, IL, IN, KY, MI, NC, ND, NE, NJ, OH, SC, TN, TX, UT, WI.
- Bachelor of Arts or Science Degree required; equivalent experience may be considered.
- Experience developing and implementing innovative care at home delivery models through collaboration with health systems, physicians and payors required.
- Experience formulating data models and metrics associated with program success required.
- Thorough understanding of Medicare regulations for home health and hospice required.
- Expertise in computer-based applications, including Google Suite and EMR Systems required.
- Moderate travel may be required.
