The posted compensation range of $43.10 - $64.11 /hour is a reasonable estimate that extends from the lowest to the highest pay CommonSpirit in good faith believes it might pay for this particular job, based on the circumstances at the time of posting. CommonSpirit may ultimately pay more or less than the posted range as permitted by law.
Responsibilities
The Supervisor of Care Coordination (CC) at Arizona General Hospital supports Care Coordination in accordance with the CommonSpirit Health Care Coordination model. The supervisor assists in the coordination of clinical resource management, progression of care, discharge planning activities, patient advocacy and clinical social work. Under the direction of the Director and/or Manager of Care Coordination, the CC Supervisor reinforces and supports strategies to achieve departmental and CommonSpirit Health goals and objectives.
This position directly supervises the CC staff with day-to-day work and to meet or exceed operational performance standards. The Supervisor ensures the implementation of CC policies, procedures and processes; assists with accreditation activities; guides department activities related but not limited to progression of care, psycho-social needs, and regulatory requirements to achieve Care Coordination program outcomes and quality metrics.
Responsibilities:
- Provides direct daily supervision and leadership to the care coordination department, including staff supervision, coaching, counseling and performance management.
- Reinforces care coordination programs and strategies to optimize patient care, resource utilization, and quality outcomes that align with system-wide Care Coordination.
- Ensures adherence to all relevant healthcare regulations, accreditation standards, and organizational policies.
- Assists with the daily operations of care coordination.
- Collaborates with internal and external stakeholders to coordinate patient care and facilitate communication across the care continuum.
- Trains and evaluates care coordination staff to ensure they are aware of and implementing system initiatives, possess the skills and knowledge necessary for effective performance and completion of required education.
- Advocates for patients' needs and rights, ensuring they receive appropriate care and support throughout their healthcare journey.

Qualifications
Minimum:
Education and Experience:
- Associates Degree in Nursing (e.g. ADN) or Masters in Social Work or equivalent education and experience
- Minimum of 2 years of clinical care coordination experience, (Utilization Management, Denial Management, Care Coordination) or 2 years of acute nursing team lead, charge or supervisory experience with working knowledge of care coordination and discharge planning
Preferred:
- Bachelor's degree in Nursing, Health Care Administration or related clinical field
- Understanding of performance improvement activities
Licensure:
- Current unrestricted AZ RN license or LCSW or LMSW from the AZ Board of Behavioral Health Examiners and working towards LCSW licensure to be completed within 3 years
Preferred:
- National certification in either of the following: CCM (Certified Case Manager), ACM (Accredited Case Manager) required or within 2 years upon hire
Training:
- Apply clinical guidelines to ensure progression of care
- Communicate effectively in writing and verbally
- Must have critical thinking and problem-solving skills
- Collaborate effectively with multiple stakeholders
- Proficient in the use of computers and multiple software programs
- Actively work within electronic health record (Epic, Cerner, Meditech, and/or Paragon)
- Understand how utilization management and care coordination programs integrate
- Work as a team player and assist other members of the team where needed
- Thrive in a fast paced, self-directed environment
- Knowledge of CMS standards and requirements
- Proficient in prioritizing work
- Highly organized with excellent time management skills
