Job Summary and Responsibilities

As a Registered Nurse (RN), you will be a pivotal healthcare professional, delivering compassionate, high-quality care that truly impacts our patients' well-being and recovery.

  • Assessment:
    Functions independently to identify, collect, and develop a database of biophysical, environmental, self-care, educational, emotional, and discharge planning needs for adult, geriatric patients in order to establish priorities for patients in accordance with the patient's age, disorder, and ethnic background. Identifies those patients requiring specialized nursing care, including preventative and rehabilitative care, those requiring physical, speech and occupational therapy services, home health aide services, and medical social services. Performs assessment functions including the patient's psychosocial, physical, functional, and resource systems, as required, to assure competent nursing care provision.
    Plans and develops care plans utilizing the nursing process according to accepted national nursing practice standards.
    Demonstrates proficiency in comprehensive assessment with OASIS data set collection as required by Medicare Condition of Participation.
    Demonstrates a working knowledge of Title 22, Medicare Conditions of Participation, and admission/discharge criteria for home health agency services.
  • Implementation:
    Provides substantially specialized nursing care, including general nursing assessment skills, IV therapy skills, medication administration, specimen collection skills, communication skills, and patient education skills. Documents all care provided according to established policy/procedure utilizing appropriate tools. Communicates with attending physician as needed to assure safe, competent care provision, including patient's general status, any untoward signs/symptoms, and the effects of treatment or lack of progress toward identified outcomes. Coordinates and manages each patient's care with the entire team, including as appropriate: physical therapy, speech therapy, occupational therapy, medical social services, and/or home health aide.
  • Other Duties:
    Maintains liaison with outside community resources to assure coordinated, ordered care is provided. Uses appropriate communication skills in data collection, nursing intervention and evaluation of care.
    Evaluates the effectiveness of nursing intervention factors in relation to intended goals or outcomes and documents patient's response to care in the medical record. Revises patient care plan based on the evaluation of effectiveness of interventions.
    Incorporates and documents teaching for patients and/or significant other with appropriate evaluation of effectiveness ofn patient's understanding. Adopts teaching approaches to the needs of the patient population, e.g. ethnic, age or other needs.
    Incorporates safety and infection control principles in teaching plans to patients and/or significant others. Assesses nursing care needs of patients and abilities of available ancillary health care personnel. Provides clinical coordination and direction to ancillary healthcare personnel. Provides direction to other members of the nursing team to ensure that patient care is administered in a thorough, safe and knowledgeable manner.
    Delegates and assigns duties for licensed vocational nurse, medical social worker and home health aide taking into consideration: a) patient's general status; b) payer-source requirements; c) environment in which care is provided; d) licensure of staff member involved; and e) degree of supervision required by the staff member involved. Participates in the orientation of new staff to the agency.
    Ensures optimum operation of the department through good attendance history and flexibility with assigned shift. Is on time for scheduled shift.
  • HIPAA:
    Basic expectations of the Employee based upon Corporate HIPAA Compliance: Patient Privacy: Demonstrates work place practices that maintain patient privacy and confidentiality. Consistently follows regulatory, JCAHO, Dignity Health, and Hospital policy requirements relating to the disclosure of and/or access to Protected Health Information (PHI) and sensitive information. Review Network Usage Policy and signed acknowledgement form annually. Patient Information Security: Demonstrates work place practices that maintain the security of protected health information including maintaining PHI in a secure work environment, reporting suspected breaches of PHI security to Management or Privacy Officer, and following all policies and regulatory requirements for the safeguarding of PHI

Job Requirements

Required:

  • One year of Home Health nursing experience within the past 5 years
  • An exception will be considered if an applicant has one year of acute care Medical/Surgical, Telemetry, and/or ICU nursing experience within the last five years. Only when adequate staffing allows or
  • Public Health, Home Health Nursing, or Hospice Experience
  • Registered Nurse: CA, upon hire
  • Basic Life Support - CPR, upon hire. We accept American Heart Association
  • Driver's License: CA, and Auto Insurance, upon hire

Preferred:

  • Public Health, Home Health Nursing or Hospice experience
  • Bachelor's or other in related field
  • Second language in Spanish
  • Knowledge of Medicare, Medi-Cal, private insurance and local community
