The posted compensation range of $60.22 - $79.24 /hour is a reasonable estimate that extends from the lowest to the highest pay CommonSpirit in good faith believes it might pay for this particular job, based on the circumstances at the time of posting. CommonSpirit may ultimately pay more or less than the posted range as permitted by law.
Job Summary and Responsibilities
As a RN Care Coordinator, you will be a central figure in patient care, seamlessly navigating the healthcare journey to achieve optimal outcomes and an exceptional patient experience.
Every day, you will strategically assess, plan, and facilitate comprehensive care across the continuum, expertly advocating for patients while collaborating with physicians, nursing, departments, insurers, and post-acute providers to ensure timely, high-quality transitions.

- The Care Coordinator is responsible for assessing planning facilitating and advocating for options and services through a continuum of care from point of contact through discharge on assigned patients.
- The Care Coordinator performs this role in such a manner as to meet the individuals health needs while promoting quality and cost effective outcomes. The positions emphasis will be on care coordination communication and collaboration with nursing physicians departments within the medical center insurers and post-acute service providers to pace the care toward optimal outcomes within the appropriate level of care.
- The Care Coordinator will advocate for the patient and family by identifying and valuing patient choice spiritual needs cultural language and socioeconomic barriers to care transitions. In addition the Care Coordinator will protect confidentiality while striving to achieve high levels of patient satisfaction.
- The Care Coordinator consistently conducts the utilization review process and the discharge planning process as required by hospital policies standards of practice and Federal and State regulations.
Minimum:
- Graduate of an accredited school of nursing.
- Minimum of two (2) years of acute hospital clinical experience, or
- A Masters Degree in nursing with a concentration in Case Management or Nursing field in lieu of one (1) year of experience.
- Must possess and maintain a current Registered Nurse license with the state of California
- Basic Life Support - CPR (BLS-CPR) within 90 days of hire
Preferred:
- Bachelor's Degree in Nursing (BSN) or related healthcare field.
- At least five (5) years of nursing experience.
- Certified Case Manager (CCM) certification, or
- Accredited Case Manager (ACM) preferred
Please be advised that this location requires quarterly American Heart Association (AHA) Resuscitation Quality Improvement (RQI) competency certification post-hire. This ongoing competency is required through resources internal to the facility; outside RQI competency resources are not acceptable for CommonSpirit Health compliance requirements.
