The posted compensation range of $28.10 - $41.80 /hour is a reasonable estimate that extends from the lowest to the highest pay CommonSpirit in good faith believes it might pay for this particular job, based on the circumstances at the time of posting. CommonSpirit may ultimately pay more or less than the posted range as permitted by law.
Responsibilities
The Staff LVN is an experienced practitioner who demonstrates in-depth knowledge of nursing and patient care, and has the ability to practice independently within the confines of their educational preparation, physician orders, and as assigned and supervised by a registered nurse.
- Demonstrates basic technical nursing skills within the scope of practice (as documented in agency skills checklist)
- Utilizes the nursing process to provide safe, therapeutic care for patients:
- Assessment, Planning in collaboration with RN, Implementation, and Evaluation
- Documentation of the nursing process reflects a proficient level of clinical practice.
- Implements an established plan of care, follows frequency, works toward established goals, and reports any necessary changes.
- Collaborates, cooperates, and coordinates with RN Case Manager/supervisor to assure effective, consistent management of clients. Works as a collaborative member of the treatment team and reports any changes in the client
status to the case manager/supervisor promptly.
Qualifications
Minimum Required:
At least one year of experience in Home Health is required.
California Licensed Vocational Nurse license
Basic Life Support (BLS) - we accept the American Heart Association.
- CA Driver’s License
- Must maintain proof of Car Insurance.
Demonstrates ability to work within a case-managed model of patient-care delivery.
Demonstrates proficient intermediate nursing skills.
Demonstrates a caring, compassionate attitude toward others.
Ability and willingness to actively participate in agency task forces and committees.
Demonstrates ability to organize and prioritize work efficiently.
Demonstrates clear, practical written and oral English skills, and objective, active listening skills.
Reacts calmly and efficiently in emergencies.
Excellent interpersonal skills and the ability to work with others across all agency departments.
