Community Health Worker
The posted compensation range of $23.00 - $32.66 /hour is a reasonable estimate that extends from the lowest to the highest pay CommonSpirit in good faith believes it might pay for this particular job, based on the circumstances at the time of posting. CommonSpirit may ultimately pay more or less than the posted range as permitted by law.
Responsibilities
The Community Health Worker (CHW) will be responsible for assisting patients and their support team, to navigate and access community services, other resources, and adopt healthy behaviors. The CHW in collaboration with Enhanced Care Management supports the healthcare system providers and staff through an integrated approach to care management and community outreach. The CHW will provide social support, informal counseling, and advocate for the health needs of individuals and their community. Community outreach and collaboration with community agencies are essential to this role. The CHW will participate in the process of collecting and maintaining health-related social needs
screening data.
- Is a role model for the Hospital's Patient Satisfaction effort, i.e. demonstrates AIDET principles when interacting with customers, subordinates and colleagues.
- Responsible for establishing trusting relationships with patients, and their support team, while providing general support and encouragement.
- Based on social needs screening results, provide ongoing follow-up, basic motivational interviewing and goal setting with patients/support team
- Follow-up with patients via phone calls and visits to other settings where patients may need assistance.
- Follow up with patients should be on-going from initial identification through closure. Help patients reach personal goals and attend appointments, as needed. Provide referrals for services to community agencies as appropriate.
Qualifications
Minimum:
- A minimum of one year experience in a healthcare setting working with patients, healthcare services, medical groups and payors required.
- A minimum of 1 year experience in community outreach, advocacy work within the healthcare/community.
- High School Diploma
- Completion of Community Health Worker certification within 6 months of hire
- Ability to use Google Workspace.
- Demonstrate ability to navigate the internet and Windows based software applications.
- Able to provide culturally appropriate service.
- Ability to build effective relationships with patients, staff, community agencies and community members.
- Knowledge of healthcare payors, medical groups and services. Understanding of medical terminology.
- Good written and verbal communication skills.
Preferred:
- Associate Degree preferred, in a healthcare-related field preferred
- California Driver's License
- Bilingual Spanish or Hmong speaker with the ability to read and write preferred.
