The posted compensation range of $18.51 - $26.15 /hour is a reasonable estimate that extends from the lowest to the highest pay CommonSpirit in good faith believes it might pay for this particular job, based on the circumstances at the time of posting. CommonSpirit may ultimately pay more or less than the posted range as permitted by law.
Job Summary and Responsibilities
At CommonSpirit Health at Home, we strive to embody our mission of delivering hope and healing to those we serve. As a Team Coordinator - Virtual Support (TC) you’ll engage in a range of exciting responsibilities for Home Health and Hospice, including but not limited to:
- Processing approved physician orders.
- Managing referrals.
- Tracking documentation requiring a physician’s signature using organization standard document management software and processes.
- Escalating per organization standard process when signatures are not returned in a timely manner.
- Assists with monthly billing; ensures orders are received prior to insurance timely filing deadlines.
- Timely processing of designated reports, summaries, and coordination notes to physicians and facilities required to comply with home health and/or hospice CoPs.
- Ensuring discharged files in EMR are complete.
Candidates must reside in one of the approved remote worker states: AR, AZ, CO, FL, GA, IA, IL, IN, KY, MI, NC, ND, NE, NJ, OH, SC, TN, TX, UT, WI
- High School diploma or equivalent.
- 2 years of customer service experience in a customer service role with effective verbal and written communication skills. 1 year of customer service preferred.
- Knowledge of medical terminology, home health and/or hospice regulatory requirements, and HIPAA compliance standards preferred.
- Experience with EMR and scheduling systems (for schedulers) or document tracking systems (for medical records) preferred, with Homecare Homebase strongly desired.
- Strong attention to detail and accuracy, with the organizational skills to prioritize workload based on patient needs, manage high volumes of scheduling activity, and adapt quickly to urgent changes.
- Effective verbal and written communication skills.
- Ability to interact professionally with clinicians, patients, caregivers, physicians, and external offices.
- Ability to work both independently and collaboratively as part of a multidisciplinary team.
