The Quality Coordinator is responsible for reviewing and approving all patient information provided by licensed professionals during essential care milestones—including Start of Care, Re-certification, Resumption of Care, Evaluation visits, and Discharge. This role promotes high-quality patient care by maintaining strong relationships with referral sources, patients, caregivers, and staff. Additionally, the Quality Coordinator assists with staff training and ensures compliance with organizational standards and regulatory requirements.
Essential Key Job Responsibilities
- Support CommonSpirit Health at Home’s mission to make the healing presence of God known by improving the health of vulnerable populations and advancing social justice, aligning with the values and strategic goals of our parent health system.
- Uphold the Corporate Compliance Program, including safeguarding the confidentiality of HIPAA-protected health information and client data.
- Review and process clinical assessments and care plans promptly, ensuring accuracy, consistency, and appropriate documentation, including assessments at all required time points.
- Follow up with clinicians on assessments that cannot be processed due to documentation deficiencies.
- Confirm proper documentation for patients transferred to inpatient facilities.
- Complete activities necessary for timely plan of care completion, including processing supplies, medications, and allergies.
- Ensure the patient’s plan of care is accurately completed and reassessed in response to significant changes or physician requests.
- Assist the Director of Clinical Services with orientation, ongoing training needs, and staff development.
- Monitor infection control reports and incident reports, ensuring appropriate follow-up.
- Provide administrative support during on-call hours to assist with intake, evaluation review, scheduling, and visit management.
- Perform patient visits as directed by management, if required.
- Perform other duties as assigned.

Qualifications
Candidates must reside in one of the approved remote worker states: AR, AZ, CO, FL, GA, IA, IL, IN, KY, MI, NC, ND, NE, NJ, OH, SC, TN, TX, UT, WI
- Graduate of an accredited nursing program; active RN license in the state of practice.
- Minimum of one year of nursing experience within home health or hospice in the past 36 months. Prior experience in review or Quality Improvement (QI) roles
- OASIS certification
- Medicare home health experience
- Effective problem-solving, strong organizational skills, ability to prioritize multiple tasks, work well under stress, and communicate effectively verbally and in writing. Proficient with computers and detail-oriented.
