The posted compensation range of $100,000 - $130,000 annually is a reasonable estimate that extends from the lowest to the highest pay CommonSpirit in good faith believes it might pay for this particular job, based on the circumstances at the time of posting. Responsibilities

The Physical Therapist, Travel provides high-quality, compassionate, and ethical care to patients in their homes, across various locations nationally.  This position will work independently and collaboratively with the interdisciplinary team to deliver care to patients who are rehabilitating, managing chronic conditions, or are in the end stage of life.  This position requires a highly skilled and adaptable Physical Therapist with strong clinical, communication, and organizational skills.

Assess, plan and implement specific treatment programs for individual patients and patient groups according to the principles and practices of physical therapy. Provides patient care/service based on identified needs/developmental level for all patient populations. Care/services includes recognition, monitoring and intervention as appropriate for age-specific and/or other individual patient needs. 

  • Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patients served.
  • Evaluates assigned patients within the guidelines established by the physician and in compliance with departmental policies and procedures.
  • Implements competent treatment and performs regular reassessments for the purpose of treatment modification, to maximize patient’s potential for functional independence, all in accordance of departmental standards.
  • Completes all required documentation and maintains pertinent records of all assigned patients, in order to be in compliance with Joint Commission, CARF, Medicare, and all other regulatory requirements.
  • Demonstrates the ability to be efficient and productive by organizing job duties and responsibilities.
  • Maintains effective communication regarding Therapy programs and departmental issues.
  • Maintains equipment and environment in a safe and orderly manner.
  • Demonstrates a commitment to professional growth and competency.
  • Other duties as assigned.


Qualifications
  1.   2. Clinical Care: Provides direct nursing care to patients in their homes, following established care plans and physician orders.  This includes, but is not limited to: assessment, medication administration, wound care, teaching, and emotional support.  Accurately documents all care provided in a timely and accurate manner.
  3. Compliance: Adheres to all applicable federal, state, and local regulations and guidelines, including HIPAA, OSHA, and infection control protocols. Maintains confidentiality of patient and employee information.
  4. Collaboration: Collaborates effectively with physicians, other healthcare professionals, patients, families, and caregivers.  Participates in care coordination meetings and communicates effectively regarding changes in patient condition.
  5. Case Management:  Manages a caseload of patients, meeting productivity expectations, prioritizing care needs and ensuring seamless transitions of care.  Proactively identifies and addresses potential issues or concerns.
  6. Documentation:  Completes all required documentation accurately and timely, including but not limited to: electronic health records (EHR), visit notes, care plans, and other required forms.
  7. Professional Development:  Maintains current knowledge and skills through continuing education and professional development activities.  Actively seeks opportunities to enhance clinical expertise.
  8. Travel:  Willing and able to travel extensively to various locations nationally, as assigned, with flexibility to adapt to different work environments and patient populations.  Responsible for all travel arrangements and expenses as outlined in the company's travel policy.
  9. Safety:  Maintains a safe work environment for self and patients, adhering to all safety protocols and reporting any safety concerns immediately.
  10. Professionalism:  Maintains a professional demeanor and appearance at all times, representing CommonSpirit Health at Home positively to patients, families, and colleagues.
  11. All other duties as assigned
