Overview

The Supervisor, Health at Home Navigators (HHN) provides direct supervision to a group of Health at Home Navigators across several markets, a provides support to Health at Home Agency Directors and Care Management Directors.

This position requires 50% - 75% travel.


Responsibilities

  1.   2. Adheres to the Corporate Compliance Program, including confidentiality of HIPAA protected health information.   3. Interviews, hires, and manages complex onboarding processes 
  4. Manages direct reports responsible for all HR processes
  5. Creates connections with HHNs and hospitals  through mentorships and national support.
  6. Facilitates HHN calls and site visits across all markets.
  7. Supports local market directors and Directors of Care Management in implementing and evaluating the effectiveness of the HHN role.
  8. Facilitation HHN implementation calls across all new markets.
  9. Addresses opportunities and barriers based on experience across diverse markets.
  10. Assists in creating and navigating reports lists in EMR
  11. Facilitates support through mentorship and HHN group meetings
  12. Creates, updates, and disseminates new educational materials, including orientation manuals.
  13. Creates and monitors KPIs to demonstrate the HHN’s value to hospitals and leadership.
  14. Drives the growth of the HHN program/role, creating and ensures the use of up-to-date data

dashboards and scorecards.

  1. Oversight of data distribution to appropriate stakeholders.
  2. Communicates with the CSH analyst the needs of new HHN’s for data collection and solve for and mitigate data collection barriers.
  3. Assists with Contracts and A2’s
  4. Hospital onboarding with input from Hospital CC.
  5. Supports the navigation of complex relationships with contractual labor and shared oversight to ensure the success and expansion of the Health at Home Navigator program. 
  6. All other duties as assigned

Qualifications

Candidates must reside in one of the following states:  AR, AZ, CO, FL, IL, IN, KY, MI, ND, NE, OH, SC, TN, TX, WI.

Active RN license in the states serviced.

At least 3 years of experience as an RN. 

  • Understanding how healthcare systems function, including insurance, funding, and different service providers. 
  • Knowledge of relevant laws, policies, and ethical guidelines governing home healthcare. 
  • Familiarity with common medical conditions and treatments to effectively communicate with patients and families. 
  • Awareness of local and regional resources, including social services, support groups, and other relevant organizations. 
  • Understanding the stages of case management, including assessment, planning, implementation, and evaluation.
