Job Summary and Responsibilities

As our Community Health Worker, you will be a vital link between individuals and essential community resources, actively working to reduce health disparities and improve access to support services.

Every day, you will proactively engage with clients, assessing their needs and identifying pathways to care and support. You'll connect individuals to relevant health, social, educational, and employment resources, providing guidance and practical assistance to overcome logistical barriers.

To be successful in this role, you will possess a profound understanding of diverse community needs, exceptional problem-solving abilities, empathy, and resilience. Your dedication to empowering clients and removing barriers to care will be paramount.

  • Conduct basic health screenings including but limited to:

    - Blood pressure checks

    - Blood glucose testing

    - BMI/weight assessments

    - Cholesterol screenings (if applicable)
  • Educate participants about their results and offer guidance on next steps.
  • Provide culturally sensitive health education and materials.
  • Refer community members to local clinics, primary care providers, and social services.
  • Maintain accurate documentation of screenings, referrals, and encounters.
  • Assist in scheduling follow-up appointments or transportation, if needed.
Job Requirements

Required

  • Must have completed an accredited Medical Assistant, Phlebotomistf or LVN program.   Will accept new graduates or a minimum of one-year experience working in the medical field
  • License Vocation Nurse Program
  • Medical Assistant Certification
  • Licensed Vocational Nurse
  • Phlebotomist: CA


Preferred

  • Experience providing direct services or outreach in underserved communities.
