Responsibilities

As a Nurse Practitioner with VNA Health at Home and Hospice, you will provide face-to-face assessment services to hospice patients to assist the Director, Hospice Clinical Services and Hospice Team to determine each patient’s continuing eligibility for the hospice program. May provide service in various settings, including the patient’s home, a physician’s office, the inpatient hospice facility, etc.

Provides care following the plan of treatment and patient care plan to assist in determination of hospice eligibility.

Establishes a care plan for continued needs based on the client assessment.

Reports changes in patient condition to appropriate personnel and the attending physician. 

Provides patient care education, counseling, medication advice, instructions and follow-up to clients and other caregivers. 

Participates in coordination of services with other disciplines and community resources as needed. 

Evaluates and revises care plans/assessment sheets based on changes in patients and/or the environment.

Assists with the development of educational programs in the Hospice program for diverse age groups (i.e., infancy, childhood, adolescence, adult and geriatric).


Qualifications
  • Graduate of an accredited school of nursing.
  • Currently licensed to practice in the state serviced, with Masters in Nursing
  • One year of nursing experience required.
  • One year of hospital nursing experience preferred.
  • Must obtain and maintain current CPR Certification.
  • Compliant with all organization, local and state regulatory requirements.
