Job Summary and Responsibilities

As our Clinic LPN (Licensed Practical Nurse), you will be a vital member of our healthcare team, contributing to the success of our practice by demonstrating exceptional customer/patient focus, effective communication, and professionalism. You will play a key role in providing comprehensive, high-quality patient care within a dynamic clinic setting at Community Memorial Hospital. Our LPNs are integral to assessing, planning, implementing, and evaluating care for diverse patient populations and their families. This direct patient care role offers a unique opportunity to grow your medical career.

Every day you will perform essential nursing duties, including general and specialty-specific nursing care, obtaining and documenting detailed health histories, vital signs, and health risk factors. You will execute prescribed treatments, administer medications, perform routine tests like injections, treat wounds, and manage bandages. A core part of your role involves patient education on health maintenance and acute/chronic condition management, along with meticulous documentation in medical records. You'll actively participate in multidisciplinary teams to enhance patient care processes and track quality assurance data for optimal acute and chronic care management.

To be successful in this role, you will need a current Iowa LPN license and a Basic Life Support (CPR-BLS) certification. We are seeking a graduate of an accredited practical nursing program with at least one year of related experience in a clinic, specialty clinic, or acute hospital setting. Strong foundational knowledge in nursing procedures, excellent communication skills (both written and oral), basic math proficiency, and computer literacy are essential. You must be adept at problem-solving, data analysis, and coordinating tasks while adhering to federal and state regulations, practice guidelines, and our high standards of ethical and religious directives.

  • Deliver Direct Patient Care: Perform general and specialized nursing care, including administering injections, performing routine tests, treating wounds, changing bandages, and executing prescribed treatments and medical interventions following established clinic workflows and guidelines.
  • Patient Assessment & Documentation: Obtain and document health history, chief complaint, vital signs, and health risk factors; assess disease conditions; and diligently record patient assessment and intervention data using medical record systems.
  • Patient and Family Education: Educate patients and their families on health status, health maintenance, and the management of both acute and chronic conditions.
  • Team Collaboration & Compliance: Participate in multidisciplinary teams to improve patient care processes, ensure patient confidentiality, obtain consent for care, and adhere to all practice guidelines, standards of care, and federal/state laws and regulations.
  • Monitoring and Evaluation: Monitor and document treatment progress and patient response, track quality assurance data, and contribute to proactive acute and chronic care management.

We are offering up to $5,000 as a Sign-on Bonus as well as up to $8,000 in Educational Assistance to new hires who meet the eligibility requirements.
