The posted compensation range of $49.54 - $73.70 /hour is a reasonable estimate that extends from the lowest to the highest pay CommonSpirit in good faith believes it might pay for this particular job, based on the circumstances at the time of posting. CommonSpirit may ultimately pay more or less than the posted range as permitted by law.
Job Summary and Responsibilities
As our Clinical Pharmacist, you’ll help our doctors and nurses safely prescribe and dispense medications to help our patients feel better and recover from their illness or injury.
Every day, you’ll build trust in these interactions by reviewing every prescription in your queue. We never rely entirely on the computer to check for safety; we always use an expert for the final check. That’s you. You’ll review prescriptions for potential drug interactions, correct dosing, patient allergies or anything that looks unusual in your opinion. To excel in this role, you must enjoy learning new concepts and researching information in real-time to provide solutions for our patients. You should also continually look for ways to go above and beyond for your patients, delivering an exceptional experience along with exceptional care.
Essential Key Job Responsibilities:
Demonstrates ability to document clinical activities and interventions such as consults, patient teachings and other patient specific information as defined by policy and procedure.
Interprets medication orders and/or prescriptions for drug interactions, appropriate drug selection and appropriateness.
Accurately enters / verifies the order or prescription into the computer system, labels, and dispenses medications in a timely manner.
Communicates drug information to patients and health professionals.
Resolves drug related problems by consulting with other health care professionals to achieve appropriate patient outcomes.
Unclear orders written by a licensed provider are clarified with the licensed provider.
Assists the licensed provider in appropriate drug selection and therapeutic regimens.
Achieves and maintains an adequate basic knowledge base to provide clinical services for all age groups served.
Provides basic clinical services as required by policy and procedure.
Responsible for technician activities, performance, provides direction and guidance as needed.
Participates in the orientation, instruction and training of pharmacy students and interns, pharmacy technicians, residents, pharmacists and other health professionals.
May serve as the Residency Program Director or Coordinator if applicable to their location.
Performs duties in documented accordance with requirements of all regulatory and governing bodies.
Provides patient counseling and drug information and documentation as appropriate.
Job Requirements
Education Required
Bachelor of Science in Pharmacy (B.S.) ORDoctor of Pharmacy (Pharm D) degree from a Board approved, ACPE Accredited College (pdf) of pharmacy program, OR
Graduated from a college of pharmacy program outside the U.S. and have obtained full certification from the Foreign Pharmacy Graduate Equivalency Committee (FPGEC)
Licensure/Certification Required
Licensed Pharmacist in the State of Texas through Texas State Board of Pharmacy (TSBP)
Basic Life Support (BLS) certification (through an approved AHA program)
Experience Preferred
Two (2) Years of experience as a Clinical Pharmacist in an acute care setting.
Master’s degree in business administration or similar field of study
Completion of pharmacy administration residency
Completion of pharmacy (PGY-1) residency
Where You'll WorkSt. Luke’s Health - Memorial Hospital in Livingston, TX, is located just off Highway 59 south. The 66-bed hospital provides more than 60,000 patient services each year, with half of those services being offered in the Level 4 Trauma-Designated Emergency Department.
