As a Social Worker at CHI St. Vincent, you will be a part of a dynamic team that works together to provide comprehensive care coordination for acute care patients to ensure a safe, timely, and appropriate discharge.

Every day you will be helping to care for patients and families dealing with social and mental health issues, chronic and debilitating diseases, and ethical situations requiring social work knowledge and expertise that will allow them to transition safely to the next level of care.

Responsibilities
  • Uses social work expertise to provide care management across the continuum for identified highest risk (complex) patients.
  • Proactively acts as a patient’s advocate, responding to and working to resolve concerns
  • Provides health promotion activities and education for patients
  • Optimizes patient and family independence through providing education and links to community resources
  • Works with families and patients on needs that may or will affect the patient’s health, including transportation issues, financial concerns, end of life planning, etc.
  • Identifies potential barriers related to patient’s home setting and self-care/management needs and works with patient (and caregiver) to resolve issues

Qualifications
  • Licensed Certified Social Work
  • Licensed Master Social Work
  • Licensed Bachelor of Social Work with minimum of 5 years medical social work experience
  • A current unencumbered Social Worker license to practice in the State of Arkansas is required
  • Experience in a medical setting preferred
