Job Summary and Responsibilities
As our Community Health Worker, you will be a vital link between individuals and essential community resources, actively working to reduce health disparities and improve access to support services.
Every day, you will proactively engage with clients, assessing their needs and identifying pathways to care and support. You'll connect individuals to relevant health, social, educational, and employment resources, providing guidance and practical assistance to overcome logistical barriers.
To be successful in this role, you will possess a profound understanding of diverse community needs, exceptional problem-solving abilities, empathy, and resilience. Your dedication to empowering clients and removing barriers to care will be paramount.
- Conducts classes within the defined curriculum.
- Keeps accurate class logs and submits information to the Manager/Supervisor.
- Conducts classes in an orderly and timely manner. Starts promptly and finishes on time.
- Maintains competence and instructional skills through attendance of continuing education programs or specialty certification.
- Provides maternity tours at designated locations and times.
- Reviews/revises curriculum with Manager/Supervisor as needed.
Required
- High School Graduate General Studies, upon hire or
- High School GED General Studies, upon hire and
- None, upon hire
Preferred
- experience in the field of education or
- obstetric experience as CNA, OB Tech or LPN or
- inactive RN with previous obstetric experience
