Job Summary and Responsibilities
The Pharmacy Coordinator Ambulatory Clinic role is responsible for interfacing with all members of the ambulatory care team, pharmacies, payers and pharmacy benefit representatives and pharmaceutical industry medication assistance programs, in order to secure access to prescribed medications. The critical tasks of this role may include but are not limited to 1) medication prior authorizations 2) support ambulatory care pharmacy services with improving patient access to medications, and 3) clerical tasks to facilitate and improve patient access to medications. Distribution of critical tasks are determined in collaboration with supporting clinic needs and requests. Day-to-day activities include fielding phone calls, scheduling patient appointments, gathering and recording care plan notes and lab results, documentation of patient encounters in the electronic health record (EHR), providing appointment reminders to patients, and answering basic patient questions. All tasks and interactions will be documented in the EHR and/or intervention application.
  • Gather required information and prepare initial assessment based on their providers’ prescribed medications for patients, including those that are unable to access or have a hardship to continue to access preferred medications



    • Research and identify potential options for the patient to secure access which may include, but are not limited to:


      ◦ Prior authorization


      ◦ Drug Assistance Program (DAP) application and enrollment


      ◦ Copay discount cards or equivalent discount program


      ◦ Foundation or grant funds available


  • Identifies patients who require review by an Ambulatory Care Pharmacist
  • Respond to provider, Ambulatory Care Pharmacist and patient self-referrals by meeting with patients to assess their medication needs and identify programs and resources to assist with ensuring access to the medication.
  • Assist patients with understanding their insurance benefits (e.g. out-of-pocket, deductible and copay expenses) and serve as a liaison between the patient and the medication assistance programs/providers.
  • Manages the renewals of medications and eligibility of patient assistance programs including phone follow-up with patients, pharmaceutical companies and providers.
  • Work with medication manufacturers to compile/maintain up-to-date assistance program requirements.
