Job Summary and Responsibilities
As our Pharmacist Resident - Second Year, you will participate in an intensive, specialized post-graduate training program designed to further develop your advanced knowledge and clinical skills for providing effective pharmaceutical care.
Every day, you will function as a key clinical pharmacist, engaging in various activities across clinical, operational, and administrative domains to apply your insights to complex patient situations. This role involves managing medication-related care for a diverse patient population, contributing to your specialized expertise and preparing you for professional credentialing.
To be successful in this role, you will demonstrate a deep commitment to advanced learning and specialized development, possess a strong foundation of clinical understanding, and exhibit a proactive approach to patient care challenges. Your dedication to refining specialized skills and contributing within a dynamic healthcare environment will be essential for program completion.
- Participate in all aspects of comprehensive clinical pharmacy services including rounds with medical teams, pharmacokinetic consultations, parenteral nutrition consultations, anticoagulation management, antimicrobial stewardship monitoring, medication reconciliation, discharge counseling, documentation of clinical interventions in the identified data base, and compliance with clinical pathways and protocols
- Performs all activities associated with meeting the goals and objectives of the American Society of Health-System Pharmacists (ASHP) accredited PGY2 residency program.
- Process prescriber orders in the identified pharmacy information systems in an effective, efficient, timely, safe and accurate manner to ensure the appropriateness of the indication, dose/frequency, route, interactions, dose adjustment, physical and therapeutic incompatibilities, side effects, duplicate therapies, and cost-effectiveness. Evaluate need for all non-formulary medication required.
- Provide direct oversight and timely and accurate verification of final product for all aseptic parenteral and non-parenteral medication preparation, dispensing and distribution in compliance with state, federal statutes and hospital procedures.
- Ensure doses are filled and delivered within established departmental guidelines in a timely and safe manner.
- Identify and evaluate adverse drug reactions and medication errors
Job Requirements
Required
- Bachelors Of Science Graduation from an Accreditation Council for Pharmacy Education (ACPE) approved school of pharmacy including didactic courses and experiential rotations in the clinical aspects of pharmaceutical care, upon hire or
- Masters Of Science Graduation from an Accreditation Council for Pharmacy Education (ACPE) approved school of pharmacy including didactic courses and experiential rotations in the clinical aspects of pharmaceutical care, upon hire or

- Successful completion of a First year residency program (PGY1)
- Pharmacist: KY, within 90 Days and
- Basic Life Support - CPR, upon hire
