The posted compensation range of $27.97 - $41.60 /hour is a reasonable estimate that extends from the lowest to the highest pay CommonSpirit in good faith believes it might pay for this particular job, based on the circumstances at the time of posting. CommonSpirit may ultimately pay more or less than the posted range as permitted by law.
Job Summary and Responsibilities
This position is responsible for providing direct clinical services to child, adolescent, adult and/or geriatric patients and families to address interpersonal and family systems issues that cause emotional distress, as well as to provide discharge planning.These include:group therapy; family therapy; individual therapy; if certified, sexual offender therapy; if credentialed, play therapy or EMDR; psycho education, discharge planning and coordinating with community, government and mental health agencies to obtain aftercare services necessary to reduce and prevent relapse.
-Assumes personal responsibility for maintaining professional development and ensuring all required CEU’s are completed and license is action.
-Utilizes knowledge of professional social work in collecting and reviewing data collected from patients, their families, outside sources such as school & outpatient therapist to assess the patient’s psychological, social and discharge needs.
-Records and communicates data to physician and interdisciplinary treatment team. In collaboration with physician and interdisciplinary treatment team prioritizes patient’s needs and develops plan of care which is developmental and age specific, individualized with specific, measurable goals and interventions to address the identified needs. Gives rational for prioritization and /or deferment of problems.
-Demonstrates proficiency and skill in providing therapeutic interventions such as family, individual and group therapy.
-Maintains accurate documents of interventions, therapies and education. Instructs patient and/or family members according to their identified need and interdisciplinary plan of care. Communicates with the patient, family, physician treatment team and other health care providers regarding patient identified needs including discharge planning and actions.
-Demonstrates effective on going observation and reassessment of patient/family and their response to the interventions and changes in condition.
-Maintains patient/family confidentiality and provides sensitive, respectful and compassionate customer service at all times.
-Documentation: Maintains accurate documents of interventions, therapies and education. Documents all interventions within given time period.

Job Requirements
Minimum Requirements:
-Masters Degree in Social Work or related field
Must possess at least one of the following licenses:
-Licensed Clinical Social Worker: KY (LCW
-Professional Counselor: KY (PROFCOUN:KY)
-Licensed Marriage and Family Therapist/Counselo: KY (LMFT:KY)
