Accountability and authority for the development, implementation and evaluation of outcomes management programs for the defined patient population.  Utilizes a variety of skills to educate, consult and assist the patient care team in achieving the highest quality and best possible outcome for the patient, in a cost effective manner.  Collaborates with a patient care team, which includes but is not limited to physicians, Radiology and nursing staff, business and financial services, utilization management and quality management; Utilizes a scientific approach in the analysis of patient care, identifying opportunities for quality improvement and cost reduction based on current research findings supported in literature. is also responsible to oversee and coordinate the workflow of the Radiology Nurses, for inpatient and outpatient services


Responsibilities

Clinical

  1. Collaborates with the patient care team to develop critical pathways based on research findings.
  2. Responds to the patient care team member’s request for consultation to evaluate complex patients’ progress along critical pathways, intervening and facilitating optimal patient outcomes.
  3. Demonstrates clinical expertise in assigned patient population, role-modeling application of clinical practice standards.
  4. Provides supervision to the nursing supervisor and staff nurses. 
  5. Promotes positive group dynamics as leader and team member. Serves as a clinical resource to staff members by providing coaching to all nursing staff.
  6. Collaborates with the Radiology management and nursing to create goals and objectives to assist the staff in acquiring, maintaining or increasing competence in his/her role responsibilities, including designated mandatory in-service requirements for all staff. Serves as clinical resource when opportunities for improvement or correcting deficiencies are noted and Develops action plan as appropriate.
  7. Performs personnel management Functions including hiring, staff orientation/development, performance evaluations and corrective action (including terminations) under hospital policy.
  8. Collaborates with Radiology management and nursing in monitoring and evaluating quality of care and patient outcomes for a specified patient population based upon standards of care as well as the findings of nursing research, hospital policies/procedures and applicable patient care protocols.
  9. Serves as consistent resource to patient care team regarding patient progress/status with critical path to promote continuity of patient care. Monitors documentation on patient record based on critical path. Participates in the development, implementation, evaluation and revision of critical paths.
  10. Maintains knowledge of current trends in nursing health care delivery. Initiates and Participates in activities designed to enhance professional development of self and staff.
  11.  Serves as a clinical resource for continuous quality enhancement including: the rules, regulations and laws of local, state and federal regulatory agencies; the accreditation standards of JCAHO; and the standards of applicable professional organizations. Promotes/Develops unit-based CQE program/ council and attends monthly service CQE meetings reporting back information as appropriate.
  12.  Reviews, Establishes and revises department policies and procedures, as necessary, to ensure continuity in compliance with JCAHO and other regulatory requirements.
  13. Attends Radiology management and nursing service education meetings and reports information to appropriate staff and modalities. Participates in development, implementation and evaluation of service education programs.

Research

  1. Facilitates the evolution of critical pathways based on scientific findings supported by research literatures.
  2. Tracks outcomes performance data, identifies significant outcomes and Assesses current clinical practice in relation to research-based literature.
  3. Plays a leadership role in TQL. Makes recommendations for change in practice to improve patient outcomes.
  4. Contributes to and/or designs research studies applicable to designated patient population.

 Education

  1. Identifies education needs and Coordinates with the patient care team to develop programs and materials for patient care team members.
  2. Communicates advance knowledge and skills related to assigned patient population at the organizational level.
  3. Improves knowledge base through attendance and participation in continuing education programs.

Administrative

  1. Role models positive guest relations and collaborative practice.
  2. Serves as a leader in the development and attainment of organizational goals as they pertain to the outcomes/nurse manager role.
  3. Analyzes the cost and quality factors related to patient outcomes and makes recommendations to the director of Radiology, the service line administrators, and the multidisciplinary group for change in clinical practice to facilitate a cost effective environment.
  4. Collaborate with the director of Radiology to assure successful implementation and progression of patients along a pathway.

Qualifications

Required Education & Experience:

*Bachelor’s Degree in Nursing (BSN) required

*Master’s degree preferred

*Five (5) years leadership experience

Required Licensure and Certification:

*Registered Nurse (RN), 

AHA Basic Life Support – CPR (BLS-CPR) within 14 - days of employment.
